Med spa payment processing fees come from two separate places. Stripe charges its published processing rate on each payment under your own Stripe agreement, and Enterprise Pay Gateway charges a monthly subscription from $49 plus a platform fee of 0.20%–0.75% on processed volume. On a $650 filler appointment on the Starter plan, the platform fee is about $4.88; everything else is Stripe's own processing cost.

Two fees, never one

Any quote that gives you a single blended percentage is hiding something. There are two costs and they belong to two different companies. Take a $650 filler appointment on the Starter plan. The platform fee at 0.75% is $4.88. Stripe's processing sits on top at its published card rate, billed by Stripe. On the Growth plan the same appointment carries a 0.60% platform fee, and on enterprise terms it falls to 0.20%–0.35%.

A $180 facial and a $4,200 device package work the same way — the platform fee is a straight percentage, so there is nothing to reverse-engineer at the end of the month.

Where fees quietly multiply

Splitting one treatment into several card taps multiplies Stripe's per-transaction component. A six-session laser package charged visit by visit is six transactions; sold as one prepaid package it is one.

Failed and re-tried membership cards do the same thing to your admin time rather than your fee line. Automatic retries with an emailed update link stop most of it.

Costs that are not fees

Chargebacks, refunds on high-value packages and staff time spent chasing balances cost far more than the percentage most practices argue about. Deposits, receipts and clear statement descriptors are the cheapest protection you can put in place.
