Payments software for chiropractic clinics
Chiropractic revenue is a mix of small repeat copays, prepaid visit packages and monthly wellness plans. Enterprise Pay Gateway covers all three with hosted checkout links, card-on-file recurring billing and prepaid packages that draw down visit by visit, with payouts to your own bank account, a fixed monthly subscription and a platform fee from 0.20% to 0.75%. Stripe bills its processing fee separately.

Typical visit
$45–$120
Adjustments are low-ticket and high-frequency, so the cost of chasing each payment matters more than the size of any single charge.
Plan fit
High
Prepaid visit packages and monthly wellness plans are the norm, which makes recurring billing and package tracking core rather than optional.
Visit frequency
2–3 per week
A patient on an active care plan may attend dozens of times a year. Card on file removes a payment interaction from every one of those visits.
Where the money leaks
Front desk collects the same copay hundreds of times
Taking a card at every visit slows the front desk and interrupts flow. A card on file, charged automatically per visit or billed as a plan, removes the transaction from the room.
Care packages are tracked by hand
Twelve-visit and twenty-four-visit packages are commonly logged on paper. Collecting the package as one payment and tracking redemption in software keeps the balance honest for both sides.
Wellness plans quietly lapse
A monthly plan only earns while the card works. Automatic retries, expiry updates and a self-serve billing portal stop churn that nobody notices until the month closes.
Insurance leaves a patient balance nobody chases
After a claim settles there is often a residual balance. A hosted invoice with a payment link recovers it faster than a statement mailed weeks later.
The four payment flows you need
- 1
New patient intake
Send a checkout link for the initial exam so the first payment is settled before the appointment starts.
- 2
Care plan package
Sell the full visit block as one product. The patient pays once and redeems visits over the plan period.
- 3
Monthly wellness plan
Bill maintenance patients on a schedule with retries on a declined card and a portal where they update details themselves.
- 4
Patient balances
Send a hosted invoice for anything left after insurance, with an optional bank-debit option for larger balances.
What chiropractic payment processing costs
Two costs sit side by side. Stripe charges card processing directly to your account — its published US online card rate is 2.9% plus 30 cents at the time of writing, with different pricing for card-present and international cards.
Enterprise Pay Gateway charges a fixed monthly subscription from $49 plus a platform fee that falls with volume: 0.75% on Starter, 0.60% on Growth, 0.40% on Pro and a negotiated 0.20%–0.35% for multi-site groups.
On $40,000 a month, a single clinic is typically on Starter or Growth: the subscription plus roughly $240–$300 in platform fees, alongside Stripe's processing charge. The pricing calculator will show your own numbers.
Prepaid care plans and what to put in writing
A prepaid package is money taken for care not yet given, so the terms need to be explicit: how many visits, over what period, what happens on cancellation and whether unused visits are refundable.
Put those terms in the checkout description. The description appears at payment and is exactly what a card issuer looks for when a charge is questioned months later.
Refund unused visits promptly when a patient moves or discontinues care. A quick refund is always cheaper than a chargeback, which carries a fee and counts against your dispute ratio whatever the outcome.
Keeping payment records separate from clinical records
Card details never reach your systems with hosted checkout, which keeps your PCI obligation at the simplest self-assessment level and keeps the sensitive part with Stripe.
Treatment notes belong in your clinical system. Enterprise Pay Gateway holds only what reconciles a payment: amount, date, product name and the email used at checkout. Do not put clinical detail in a product name or invoice description, since it appears on statements and receipts.
Frequently asked questions
- Can a chiropractic clinic keep a card on file for repeat visits?
- Yes. The card is stored with Stripe, not in your systems, and can be charged per visit or billed as a recurring plan. Tell the patient in writing what will be charged and when.
- Do prepaid visit packages work?
- Yes. Sell the block as a single product at its package price, collect it in one checkout, and track redemption as visits are used.
- Can we bill balances left after insurance?
- Yes. Send a hosted invoice with a payment link and, for larger balances, allow bank debit so the processing cost stays proportionate.
- What does it cost?
- Stripe's processing fee plus a platform fee. Enterprise Pay Gateway starts at $49 a month plus 0.75%, falling to 0.40% at higher volume.
